Online intake questionnaire
What it is
The U.S. Mission South Africa page (7 October 2026, U.S. Mission South Africa, marked as a 24 minute read) tells people who meet every eligibility rule, and who have not yet been contacted by a USRAP partner, to complete the online USRAP Intake Questionnaire if they want to be considered for referral [source].
The Embassy's own link, checked on 24 October 2026, is https://us-dos.unqorkgov.state.gov/?#/display/68a77f413cdb0fe7ab8817a8 [source]. Do not use a look-alike site.
The same page says you must complete this questionnaire even if you already submitted a Statement of Interest on the old link, if you are still interested and a partner has not contacted you to act on that statement [source].
Who should submit it
- People who meet all of the criteria on the eligibility page [source].
- People a USRAP partner has not yet contacted to start a referral or to process the case [source].
- A family group of one person still uses the family-group submit button. The Embassy FAQ says that [source].
Who may submit on behalf of someone else, and every question on the form: not stated by the Embassy pages. The Embassy says it is not necessary to have documents verified [source].
What the Embassy says happens after you submit
- Your application is reviewed by a referring organisation, Church World Service (CWS) or Amerikaners. They may contact you if they need more information [source].
- The 7 October page says the Department of State's referral partners review submissions and may ask for more information. Because of the volume of interest, you may not get a response for an extended period [source].
- If you meet the qualifications for referral, the Bureau of Population, Refugees, and Migration (PRM) evaluates access to USRAP [source].
- Not everyone is referred. The U.S. government considers eligibility, the persecution claim and background checks. The authority to grant access rests with the U.S. government, not with RSC Africa/CWS or Amerikaners [source, source].
- You are told whether you are referred or not [source]. If you are accepted, RSC Africa/CWS tells you the next steps: a pre-screen interview, security vetting, a USCIS interview and a medical exam [source].
- There is no standard or guaranteed timeline [source].
- Keep the email address you used on the form. If your address, email or phone changes after you are granted access, tell RSC Africa immediately [source].
- Case questions after processing has begun: write to SouthAfrica@cwsafrica.org and include your case number. You can track the case only after you have a case number [source].
How to know the form went through
The Embassy FAQ says to check the dashboard [source]:
- You should see "Information Entry Complete" in the status column next to each family member, after that person's form is finished.
- Then select "Submit Family Group UIQ Application" at the bottom of the dashboard.
- You should then see "Complete" under the Family Group UIQ Status.
If you do not see those statuses, the FAQ says the family group was not submitted. Finish each person and submit the group again. That includes a family of one [source].
The form is free
- Access to USRAP is free, including the questionnaire. Referral partners do not charge fees [source].
- The Embassy FAQ says referrals, applications and related services are free. Do not pay anyone who claims they can expedite the case or guarantee approval [source].
- The only official referral and USRAP implementers named in South Africa are RSC Africa/CWS and Amerikaners. Report fraud with the USRAP Tip Form on the Embassy page [source, source].
Read Avoiding scams before you pay anyone or hand over identity documents.
February 2025 email and the October 2026 page
The Embassy notice dated 13 February 2025 says: for more information contact PretoriaPRMInfo@state.gov; if you already wrote, they are reviewing messages; the programme is for consideration for refugee resettlement; access is free; report fraud to that same mailbox [source].
The 7 October 2026 page does not mention PretoriaPRMInfo@state.gov. It says you must complete the questionnaire even if you already sent a Statement of Interest [source]. It does not use the word "replaced". Both pages are still on the Embassy site. Follow the newer page for how to ask for a referral.
